Treatment is an important beginning. It is not the finish line. The weeks and months following treatment often bring new opportunities and new challenges: returning home, going back to work, rebuilding relationships, managing stress, creating new routines. Continuing Care helps clients navigate those transitions with confidence.

At Present Moments Recovery, continuing-care planning starts well before discharge, and the same team that guided treatment helps guide what comes next. The two pages below cover the pieces that matter most: the people around you, and the life you are building.

What is Continuing Care?

Continuing Care is the ongoing support that follows formal treatment. It helps clients remain connected while applying the skills learned during treatment to everyday life. Recovery is strengthened through continued accountability, healthy relationships, and personal growth.

What a plan includes

  • The next step in care: IOP, outpatient, sober living, or aftercare.
  • Ongoing supports: therapy, peer groups, family involvement.
  • A relapse-prevention plan with clear next moves.
  • Connections to the community resources that fit the person's life.

Every recovery plan is different

Some clients benefit from individual therapy, IOP, recovery coaching, alumni groups, 12-Step meetings, SMART Recovery, family counseling, faith communities, sober living, psychiatric follow-up, or ongoing medical care. Recovery plans should reflect each person's unique goals and needs, not a one-size-fits-all template.

Transitions can be challenging

Returning home often means returning to familiar environments: old routines, old friends, old stressors. Continuing Care helps clients prepare for those transitions before they happen, so the first hard day at home is not the first time anyone thought about it.

Staying connected

Long-term recovery grows stronger through connection. Clients are encouraged to remain connected with sponsors, therapists, alumni, family, physicians, recovery meetings, mentors, and healthy friends. Connection helps prevent isolation, and isolation is where relapse usually begins.

Accountability creates freedom

Accountability isn't punishment. It's support. Regular check-ins help people notice challenges early while celebrating progress along the way.

Recovery continues to evolve

Recovery at one year looks different than recovery at thirty days. Recovery at five years looks different than recovery at one year. Growth continues. New goals emerge. Life changes. Continuing Care provides ongoing support through those seasons.

There is no single timeline. Many people benefit from structured support for a year or more after treatment, gradually shifting toward community resources, meetings, and relationships that continue indefinitely. The plan should evolve as recovery grows.

Does everyone need meetings?

Not necessarily, though most people benefit from some form of regular recovery community. That may be 12-Step meetings, SMART Recovery, alumni groups, faith communities, or therapy groups. What matters most is consistent, honest connection with people who support recovery.

What happens if I struggle after treatment?

Struggling does not mean starting over, and it is never a reason for shame. Reach out early: to your therapist, sponsor, our team, or another support. Continuing-care plans include clear next steps for hard moments, and returning for additional support when needed is a sign of strength, not failure.

Can my family remain involved after treatment?

Yes, when that is right for your situation. Family counseling, family support resources, and ongoing communication can continue after discharge. Family involvement is always individualized based on each person's needs and relationships.

What community resources are available?

North County San Diego has a strong recovery community: 12-Step and SMART Recovery meetings, alumni networks, sober activities, faith communities, and volunteer opportunities. Part of continuing-care planning is connecting each client with the specific resources that fit their life.

How does Present Moments Recovery help after discharge?

Every client leaves with a written continuing-care plan built together before discharge: the next level of care, ongoing supports, a relapse-prevention plan, and community connections. Our alumni community keeps the door open, and clients are always welcome to reach out when life gets hard.
